Insurance Information:       

Pediatric Affiliates accepts most PPO and HMO insurance plans and will gladly bill your insurance company as a courtesy to you.  Please be aware that all medical costs incurred are the patients responsibility regardless of insurance.  Additionally, it is the patient's responsibility to know what is and what is not covered under their policy.  If you have any questions or if we can be of assistance to you on these matters, please feel free to contact the office manager, Pam Hankins, at (818) 883-0460 x103.
